Required Forms and Paperwork

Q: What UIL & local paperwork do I need?

A:  All incoming athletes will need a physical, acknowledgement of rules, and steroid form.  Athletes in the 7^th ,  9^th & 11^th grades must also complete a doctor’s examination on their physical.  Irregardless of grade level, if you have had a serious injury, or surgery, since your last physical, a doctor’s physical examination must be completed before participation in the new school year.  All new athletes in the high school are required to turn in the Tom Bean ISD Athletic Handbook acknowledgement page before participation.

     Transferring athletes in grades 8 and above must also complete a Previous Athletic Participation form, the UIL Acknowledgement of Rules, the UIL Steroid Policy and the Tom Bean ISD Athletic Handbook acknowledgement page.  No student will be allowed to participate in volleyball until these forms are filled out.

       Returning athletes in grades 8, 10 & 12 must complete a new UIL Acknowledgement of Rules, and a new UIL Steroid Policy, and the front of the physical form which is a medical history questionnaire. 

Q: I am a transfer student from another school.  Do I need special paperwork to participate in athletics?

A:  If you transferred from another school from the 8^th grade then your answer is yes.  Students that transfer after the first day of school must submit required paperwork (physical form, acknowledgement of rules, steroid form, and previous athletic participation form [get with coach]) and sit out the required amount of time for a varsity level sport. Volleyball players transferring before school starts usually do not have a period of time to sit out as soon as all paperwork is in order.

Volleyball Camp

Q: What do I need to bring to volleyball camp?

A: Gym shoes, a towel (if needed), water (if needed), kneepads.  T-shirts will be given out on Thursday for competition and camp pictures on Friday.
